After a transcript has been ordered, a student may authorize another person to pick it up.

A student may authorize another person to pick up the official academic transcript by using a Transcript Agent Authorization form. The transcript must be ordered through the regular ordering process: on MyUCLA or by submitting a Transcript Order form.


The authorized agent must appear in person, with photo ID, at the Registrar’s Office, and must submit the completed Transcript Agent Authorization form and a scanned print or photocopy of the student’s photo ID. In accordance with UC policy as well as state and federal privacy laws, the student’s signature is required to release an official transcript.

A transcript may not be released if the student has outstanding financial obligations to the University. The student should check MyUCLA for academic record status and holds.


The authorized agent should submit this form with documentation, and pick up the transcript in person, at the Registrar’s Office student services windows.

In compliance with COVID-19-related safer-at-home orders, Registrar forms cannot be accepted, and documents cannot be picked up, in person at this time. If the service is not available online, send the form by e-mail instead. Contact Registrar’s Office units by e-mail or through the MyUCLA Message Center with questions.
